Creating a patient questionnaire

Questionnaires allow you to collect structured information from patients before a consultation. Responses are stored against the patient record.

How to create a questionnaire

1. Click Questionnaires in the left navigation
2. Click + Create Form
3. Give the questionnaire a title and description
4. Add questions — each question can be text, multiple choice, or yes/no
5. Click Save

Sharing a questionnaire

Each questionnaire has a unique public link. Click the Copy Link button next to the questionnaire to copy the URL. Send this to the patient by email, SMS, or WhatsApp.

The link format is: app.getclinic.io/q/[TOKEN]

Patient experience

Patients can complete the questionnaire on any device without logging in. Responses are automatically linked to their patient record when submitted.

Viewing responses

Completed questionnaire responses appear in the patient's profile.
